Surgical Treatment

1. Surgical treatment

Pancreatic surgery is an extremely difficult major operation, among which pancreaticoduodenectomy, also known as Whipple operation, is the most challenging. Traditional surgery requires a large wound, and the patient's postoperative wound pain index is beyond the table, and the recovery is very hard. In recent years, minimally invasive technology has allowed us to perform major operations with small wounds, so that our patients no longer suffer. Minimally invasive pancreaticoduodenectomy with Da Vinci robotic arm is a highly difficult operation. Not every hospital or doctor has the ability and conditions to develop. It must rely on an excellent and professional pancreatic team to create excellent results and gain the trust of patients.

Under the efforts of the pancreas team led by Shin-E Wang Director and Yi-Ming Shyr Director of the Department of General Surgery of Taipei Veterans General Hospital, they are actively developing "the highly complex and difficult minimally invasive surgery for pancreatoduodenectomy with the Da Vinci robot arm". Since Taipei Veterans General Hospital has more than 1,900 cases of traditional pancreaticoduodenectomy, with the efforts of our pancreas team, we have completed more than 590 cases of pancreaticoduodenectomy with the Da Vinci robot arm Minimally invasive surgery, write almost demanding records:

     1. Discharge on 6 days after operation: Compared with the traditional surgery with large wounds, the average length of hospital stay is about 31-36 days, almost one month earlier

     2. Minimally invasive wound: the wound is as small as 3-4 cm, about 1/10 of 30-40 cm in traditional surgery

     3. Less blood loss: The amount of blood loss in surgery can be reduced to almost "zero", with an average blood loss of about 100 c.c., which is much less than traditional surgery of about 500 c.c.

     4. The operation time of the robot arm operation is short: the operation time of the robot arm operation can be as fast as less than 4 hours, only 232 minutes, which is faster than the traditional operation of 6~8 hours

     5. Suitable for elderly and high-risk patients: 95-year-old patients can be discharged smoothly. Since it is a minimally invasive surgery, it is more suitable for elderly and high-risk patients.

     6. The surgical mortality rate is low, less than 1%. The results of the overall satisfaction survey of patients with Da Vinci robot arm-assisted minimally invasive pancreaticoduodenectomy surgery are as high as "99% satisfied".

Published 67 academic papers (papers related to pancreas and pancreaticoduodenectomy), including 1 chapter in an international textbook on pancreas and 6 papers on minimally invasive pancreatic surgery with Da Vinci robotic arm. Due to the efforts of our pancreas team, Director Wang Xinyi was invited to give a speech at the International Society of Hepatobiliary and Pancreatic Surgery in Geneva, Switzerland in September 2018, and was the only representative from Taiwan to participate in the formulation of the "International" pancreatic microbiome in Miami, USA in March 2019. Create the "statute" of surgery, shine on the world stage and win glory for the country! The quantity and quality of minimally invasive pancreas surgery performed by the pancreas team of Taipei Veterans General Hospital is not only the first in Taiwan, but also has a pivotal position in Asia and the world. The pancreas surgery team of Taipei Veterans General Hospital is also the only excellent team in Taiwan that has won two bronze awards, the so-called "Symbol of National Quality" Oscars in Taiwan's medical circles.
