Be sure to write the information you want to apply for in the plan clearly.

The part that needs to be approved by the IRB (must be within the scope of the announcement, you can apply less, but you will not be able to apply if you exceed it), the information room will provide information accordingly (very important).

(2) The application process is as follows:

1. Enter the CHANGE "Application for Change of Information Application System" on the staff information portal of the hospital (<== please click). Please refer to this file for the filling example (<== please click). After filling out the information, be sure to ask the applicant and the supervisor of the unit to verify the seal, and mark down the date in 8 codes.

2. Please fill in the application form of big data research materials (the inner page needs to be signed) + declaration form (<== please click) + attachment of the IRB approved clinical trial/research certificate (including plan) + the agreement of executing certification issued by Clinical Research Subject Protection Center.

3. Please prepare the above-mentioned items, and send them by official document or send in in person to the information room engineer Yuan-Jia Chu (the host). (The engineer will then run the process to the IRB, big data center, medical record room, and medical enterprise. After approval by the department secretary and vice dean Shou-Yen Kao, the application returns to the information room, the information room can hence provide the data of the current "open application scope”).

The usages of the database and warnings

1. Precautions when applying and the period of use

• The applicant must be a staff member of the hospital
• Attach the certification of IRB review and approval issued by the TPEVGH Human Experiment Committee
• Attach the database usage plan (similar to the health insurance database application plan)
• The approved materials are used for a period of 3 years. A certificate of destruction must be provided upon expiration. If you need to extend the period of use, you need to apply again.

2. Related specifications to database usage (1)

• Comply with the "Personal Data Protection Act", "Human Research Act" and related laws and regulations. The same applies after the applicant resigns.
• The original data shall not be published or used for other purposes, shall not infringe on personal privacy rights or identify individual units, and shall not be used for other non-statistical analysis purposes. The applicant shall be held legally responsible for any damage caused to others.
• Materials may not be reproduced or provided to persons or institutions other than the research applicant.

3. Related Specifications to database usage (2)

• Within one month after publication, the soft copy of the manuscript should be submitted to the center for inspection; if not provided, the center may reject the next application.
• If there is any violation of the relevant regulations, in addition to returning all the data and providing a certificate of destruction, the center may suspend the right of the applicant and the research team to apply for the use of this database.
• The applicant shall be fully responsible for all the consequences caused by violating relevant laws and regulations.

4. The source citation of the database

• All papers written using the application materials should cite the source of "Taipei Veterans General Hospital Big Data Center". The following should be listed:
