Taipei Veterans General Hospital
Division of Clinical Toxicology & Occupational Medicine
PCC-Taiwan
The Taiwan National Poison Center (PCC-Taiwan) was founded in 1985 under the auspices of the Department of Health, Republic of China (R.O.C.) and the Taipei Veterans General Hospital. It serves the 23 million population in Taiwan and has been in operation 24 hours a day since July 1985. It is staffed by five doctors (internists), four pharmacists, two nurses, eight laboratory technicians and about three dozens of trained medical student volunteers. The PCC-Taiwan provides online information, interpretation of laboratory tests, and management options for poisoned patients. In addition, because of its affiliation with the Taipei Veterans General Hospital (VGH), one of the largest hospitals in Taiwan, the PCC-Taiwan is a referral center for poisoned patients nationwide. The PCC-Taiwan also serves as a training center for emergency physicians, clinical toxicologists, and poison information specialists. By providing readily accessible service, timely advice and expertise in the treatment of poisonings, the PCC-Taiwan has earned an excellent reputation among patients, physicians and governmental agencies.
With its affiliated toxicological laboratory, the PCC-Taiwan is also the best center in conducting toxicological laboratory analysis for acutely poisoned patients in Taiwan. It not only provides analysis of a wide array of toxicants but also offers a training program for toxicological laboratory technicians. Moreover, the PCC-Taiwan has been involved in the epidemiological investigations of various abused substances and has joined the official certification program for drug abuse screening in Taiwan since 1990. Many numerous new analytical methods such as the analysis of toluene, 1-bromopropane, dimethylacetamide, aconitine, ephedra, traizolam, bromadiolone and metformin had been developed in last two decades as well.
In addition to providing online information service and toxicological screening, the PCC-Taiwan has focused its activity on the survey and prevention of occupational poisoning exposures. In last three decades, the PCC-Taiwan has been involved in the management of several large outbreaks of occupational poisoning exposures in Taiwan. To provide a training program for occupational medicine physicians is another major task of the PCC-Taiwan. As of July 2015, more than 10 occupational medicine physicians have received 2-year training in the PCC-Taiwan and the Division of Clinical Toxicology & Occupation Medicine, which has greatly increased the manpower in the better management of occupational poisoning exposures in Taiwan.
Education and training: The main strength of the Clinical Toxicology training program at the Taipei Veterans General Hospital is its vast number of cases collected over more than 29 years of operation. These invaluable data are available for retrospective analysis and research of various acute poisonings. Agrochemical exposures and herbal poisonings in Taiwan are not commonly encountered in poison control centers in western countries. Other aspects of toxicology service such as the laboratory analysis, clinical toxinology and occupational poisonings also provide a greater opportunity for immersion yourself in the fascinating world of clinical toxicology.
Students receiving training at the PCC-Taiwan include residents, interns, medical students, pharmacists, nurses and laboratory technicians. The training course may vary from weeks to 2 years and primarily focuses on problem-based learning & situated learning.

Plastic Surgery
Clinical Elective Program for Foreign Medical Students
Nov 2015
Medical students are always told that plastic surgery is very important because we are the last real general surgeons. Unlike the other medical specialties, we do not confine our job to a single body area or link it to a specific disease. Similarly, while some operations are reasonably standardized, plastic surgery is the one in which there are more exceptions than rules. Medical students for elective clerkship training will learn and practice based on the following six core abilities: patient care, medical knowledge, practice-based learning and improvement, interpersonal and communication skills, professionalism, and systems-based practice.
- Understand the diagnosis and treatment of the common diseases related with plastic surgery.
- Learn and practice how to take integrated patient history, perform physical examinations, and write medical records through daily patient care.
- Perform evidence-based practice and search references.
- Basic surgical technique and aseptic technique.

Orthopedics
Clinical Elective Program for Foreign Medical Students
November 2015
The core practices of Orthopedics Department include traumatology, spine surgery, bone diseases and tumors, sports medicines, hand surgery, joint reconstruction and pediatric orthopedics.
The Orthopedic Department is based on but not limited to the studies on fracture traumas and treatments.
Our orthopedic trauma team utilizes the cutting-edge techniques of microsurgery on fracture treatments. Aside from that, we have remarkable achievements in the fields such as spine diseases, hand diseases, bone tumors, congenital bone diseases and restoration of motor functions.
Our spine surgery team is one among the few hospitals in Taiwan able to perform scoliosis surgeries, and have achieved thousands of successful cases so far. Aside from the common lumbar diseases, this team also pioneers in surgeries on thoracic and cervical diseases.
Sports medicine has become more and more important since the citizens, either professional or amateur athletes, engage to a lot of sports activities. Our team is second to none in Taiwan in terms of experiences on managing knee and shoulder injuries.
Congenital bone disorder has been a conundrum in medicine. Pediatric orthopedics has challenges in diagnosis, treatment as well as follow-up patient care. Our team has been devoted to the research on serious congenital scoliosis for years, hoping to shed some light on the field and to benefit the patients and their parents.
Over the decades orthopedists have tried to apply what they learned from microsurgery to the restoration of the hand or arm functions of the countless patients via ligament reconstructions and arthroscopy on wrists and elbows.
In Taiwan as an aging society, joint reconstruction has gained more significance than ever. Our team has thousands of cases each year subjected to reconstruction operations. Not only hip, knee, shoulder or elbow replacement, our team also deals with nasty infections or reconstructions well. We further employ corrective bone surgery on young patients to prevent them from the doom of total joint replacement. As a forerunner in introducing and practicing the arthroscopy in Taiwan, we have presented thousands of cases of arthroscopy to impress audiences on the international orthopedic conferences.
About half of the bone tumor cases in Taiwan are treated at our hospital. For this reason, we have established a bone bank, abundant with donated bones, for use in various reconstructions of bony defects or allograft transplants.
Last but not least, our resources are also allocated to academic researche on the innovative fields such as molecular biology and genetic/tissue engineering awaiting future clinical applications. Under the collaboration with National Yang-Ming University, we integrate the clinical and academic resources for exploration into the fields such as osteoporosis, cartilage regeneration and the application of stem cells.

The department contains 3 divisions: Otology, Rhinology-Head and Neck Surgery, Laryngology-Head and Neck Surgery.
The Otology Division provides specialized surgery in many aspects such as Tailor-made mastoidectomy with cartilage reconstruction for treatment of cholesteatoma, the minimally invasive transcanal middle ear microsurgery for chronic otitis media, laser assisted minimally invasive surgical technique for otosclerosis, Cochlear implant for the moderate to severe hearing handicapped, and the use of translabyrinthine approach in cooperate with neurosurgeon to remove cerebellopontine angle tumors. For outpatient services we have Hearing Scale Test to quickly screen the patient's hearing status, laboratory examinations such as nystagmography, vestibular function test, posturography, caloric stimulation and VEMP to explore the cause of vertigo and the balance; and the specialized services of tinnitus and aural rehabilitation to promotes desensitization therapy to achieve the best therapeutic effect.
The Rhinology-Head and Neck Surgery Division works to develop various kinds of skull base and craniofacial surgeries and to adopt compound therapy to enhance survival and quality of life for patients with head and neck cancer. We renovate endocrine paranasal sinus surgery for resection of tumors in nasal cavity, paranasal sinus and skull base, orbital decompression, dacryocystorhinostomy and cerebrospinal fluid leakage repair by using microdebrider and navigation system to make it more exquisite and safer. Radiofrequency surgery is performed to treat allergic rhinitis, chronic hypertrophic rhinitis, and hypertrophy of tongue base related obstructive sleep apnea syndrome more effectively.
The Laryngology-Head and Neck Surgery Division has accomplished the following achievements: 1. wide use of CO2 laser microsurgery for the treatment of early and moderately advanced pharyngeal cancer, laryngeal cancer and oral cavity cancer to keep speech and swallowing functions of the pharynx and larynx; 2. establishment of multi-disciplinary head and neck cancer therapeutic team to treat advanced stage head and neck cancer (including oral cancer) with chemotherapy and radiotherapy and to participate international clinical trials; 3. identification of small tumor with narrow band imaging for early diagnosis and therapy of head and neck cancers; 4. developing endoscopic fiberscopic surgery for the benign vocal fold lesions under local anesthesia in outpatient basis.

Dermatoloty
Clinical Elective Program for International Medical Students
The Dermatology Department contains 2 divisions: Dermatologic Diagnosis and Photochemotherapy.
The missions possessed at the establishment of our department are to promote adequate and high-quality patient care in clinical dermatology and to train professional dermatologists who are also willing to dedicate to education and research. Our department consists of 12 physicians, including renowned attendings, chief residents and junior residents, cooperating together to provide the best patient care and research. We offer most of our services on an outpatient basis, with more than 300 visits a day.
In addition to the general dermatology outpatient services, the Department allocates specialized laser treatment sessions especially for amyloidosis patients. An inpatient service of 16 beds is also available for the intensive treatment of severe dermatoses. We have many gadgets and devices to help patient with their skin problems, including liquid nitrogen for the treatment of warts and skin tumors, ultraviolet chamber for the phototherapy of psoriasis, and low energy level laser for vitiligo. We started to expand our cosmetic and laser dermatology services in 2002 and have different kinds of laser equipments to provide advanced cosmetic treatment, such as dye laser for telangiectasia and spider angioma, Nd-YAG laser for the removal of tattoo and some pigmented dermatosis, and Erbium-YAG laser for syringoma, amyloidosis and certain skin tumors. These treatments improve not only the appearances, but also the psychosocial well-being of the patients.
Recently, we built up the “Holistic Health Service System” in 2012, aiming to provide brand-new medical services by combining cosmetic medicine and health physical examination. We provide, in essence, all sorts of skin examinations and mini-invasive cosmetic procedures for patients looking for friendly medical environment and help patient “become beauty from inside out”!
Our faculty also has an active research arm, with prolific annual publication in some well-known journals both in Taiwan and abroad. The leading studies we have done in genetic and molecular biology of amyloidosis and psoriasis have furthered the understanding of their pathogenesis, and new approaches to the disease treatment. Studies of autoimmune bullous disease and follicular stem cell in vitro and in vivo are the projects we find exciting and would like to devote more studies in the future.

Ophthalmology
Clinical Elective Program for International Medical Students
At the establishment of the hospital, Ophthalmology was one division of Surgery Department. It became an independent unit in 1975 and evolved into Ophthalmology Department in 1989. It is the first unit in Taiwan that developed microsurgery for cataract and placed the first intraocular lens in Taiwan. It is also one of the medical centers that introduced fluorescein angiography into Taiwan and the first one experienced in pathological diagnosis in ophthalmology. During the past years, we continued to progress in three aspects, including clinical service, medical education, and research in clinical and experimental science.
In addition to general ophthalmology, we provide outpatient services in several subspecialty fields, including cornea, uveitis, oculoplasty and reconstruction, neuro- ophthalmology, glaucoma and retina. We are equipped with many computerized and digitalized instruments that provide comprehensive eye examinations: fluorescein angiography, indocyanine green angiography, computerized automated perimetry, optical coherence tomography, IOL master, and Obscan et al. We provide laser treatment to various eye diseases by using different laser modalities including argon laser, Nd-YAG laser, Diode laser and PASCAL wave scanning laser. Geriatrics and Gerontology
Clinical Elective Program for International Medical Students
Population aging is the most critical issue in both individual and societal aspects nowadays. The percentage of people aged over 60 years will increase from 11.5% at present to 22% in 2050. However, increased life expectancy and decreased fertility rate are accompanied with overwhelming health, economic and social problems.
Aging is associated with changes of body composition, organ insufficiency, and progressive physical and cognitive functional decline. Multiple chronic diseases and increased pills burden are common problem in the elderly that increased the risk of inappropriate medications and adverse effects. Furthermore, functional deterioration, emotional stress and environmental limitation that will influence the independency and quality of life among the elderly. Accelerated aging process, that indicated by geriatric symptoms, represents the status of decreased physical reserves, increased vulnerability to external stressors, thus poor health outcome and increased mortality risk. Frailty is compatible with accelerated aging that is featured by functional limitation and sarcopenia. Multiple factors have been reported to contribute to the development of frailty and sarcopenia, including altered hormone homeostasis, chronic inflammation, excessive oxidative stress and undernutrition. Therefore, early identification of frailty and sarcopenia may play an important role to prevent the related adverse health outcome for the elderly.
More than controlling chronic diseases, functional interventional program and continuous caring models in different stages of aging plays important roles that assists those elderly to live individual life at home or in the community. Seamless transition between homes, institutes, acute-care unites and post-acute care facilities are an ideal way that decreases the burden of seeking assistance and also improves their quality of life. To efficiently running these caring models, interdisciplinary team work for each individual setting is necessary. At the final end of life, how to help the elderly understating the meaning of advanced life directives, palliative care, and do-not-resuscitate decision elderly on their own decision is also an important issue in life.
Therefore, that's important to learn how to identify geriatric syndromes including frailty and sarcopenia, how to cope with multimorbidities and polypharmacy, and how to manage the seamless continuous caring models by interdisciplinary team work through this course. The main point is to improve the quality of life of the elderly, but not to increase the suffering in prolonged life expectancy.

Acupuncture
Clinical Elective Program for International Medical Students
Acupuncture, by using dry needles or moxa, has been used in Chinese society for more than 2,000 years. The evidence exits in Hwang-Ti-Nei-Jing (黃帝內經) which is the first and greatest book of Chinese Medicine and has been proved to be written around Han Dynasty, during the 1st century. It records the idea of Ying and Yang, meridians, organs function, Ching (精) Chi (氣) Shen (神) and different needles used in treatment and ways of manipulation, prevention of illness, exercise by following the nature in daily activities and so on. According to this Book, ancient Chinese doctors developed herb treatment and diagnostic rules, including visible check-up (望診) and pulse diagnosis, five elements (五行), etc. Maybe, Chinese society at then paid much attention on its tradition and ancestors. People purely kept the idea of ancient way and seemed very hard to get changed or absorb new knowledge. However, they were all in the past and no way and no reason to be influenced by the modern people now.
Puncture dry needles into specific points of body. Effective results occur with sensation of De-Chi (得氣). This offers analgesic and regulation function. Acu-points locate on 14 meridians lying over the body surface and connecting limbs and trunks with the internal organs. Not only dry needles but also moxa, cupping, electricity, laser, massage, all physical ways can give stimulation over the human body. The mechanism for it is through the neuro-endocrine system as modern language or through the Chi and Blood circulation as the ancient Chinese language.
Learning the TCM basic theory, Chinese herbal medicine, Acupuncture, Acupoints and Meridian with lecture and clinical observation.
