五、出席方式：因疫情影響，此次研討會採「現場」出席或「線上」出席
(線上出席無法認證，不給予繼續教育積分)
六、報名時間及方式：
（1）即日起至110年04月09日(五)中午12:00止。

七、繼續教育積分申請中：公務人員、西醫師、內科、護理師、專科護理師、藥師、營養師、職能治療師、物理治療師、老年醫學專科積分、家庭醫學專科積分、長期照顧服務人員積分。(依現場學分為主)

九、本研討會『不提供』停車費減免。
十、主辦單位保留變更議程及講者之權利。
十一、課程當日請攜帶健保卡簽到退，共需三次，本課程『不提供』上課證明。
十二、若學員已過早上報到時間，不保證提供課程講義，造成不便，請見諒。
十三、課程當日如遇天災、颱風等不可抗力之因素影響，則依據行政院人事行政總處公佈天然災害停止辦公及上課情形決定是否停止辦理，並不另行公告，課程延辦之日期將擇日另行公告。
十四、如當日您有身體不適、上呼吸道症狀、發燒、或其他新冠肺炎症狀、相關接觸史等，請勿前來。
